Intensive In-Home Coordinator

The Intensive In-home Coordinator will assist in the day-to-day coordination of the Intensive In-Home program and will provide brief behavioral and emotional services to children and their families within a community-based model. The coordinator is a member of a 3-person team, with a maximum case load of 12 families in providing such services; conducts skill-building activities, provides parenting education, intervenes during a crisis, provides on-call coverage within the intensive in-home team 24/7/365. Services are provided in the client’s home, school, and community settings, under the direction of the Intensive In-home Therapist.

Charlotte, NC

Case Manager – On Ramp

The A Way Home Housing Case Manager provides a variety of office and field activities to manage and monitor a rapid re-housing program for pregnant and parenting young adults. Employee will provide direct client services, complete related documentation and implement the TIP Model of case management with housing clients. Work involves providing supportive services to promote participants self-sufficiency, integration into the community, and permanency in housing, performing administrative tasks involved in the review and maintenance of a caseload of program participants.

Lead Program Coach – My Sister Susan’s House

The Lead Program Coach works as a team member and provides services for clients in the MSSH Program, our  Transitional Living program that provides counseling, vocational and educational training, independent living skills training, and other supports for pregnant or parenting youth ages 16-21. The Lead Program Coach provides direct care to residents on-site and assists them with developing independent living skills and other domains to prepare for self-sufficiency.
